Wisdom Surgery Clinic
Policies, privacy, financial information, and your new patient registration form.
These policies govern your care at Wisdom Surgery Clinic. Please read them before completing your registration form. Last updated: April 2026.
Wisdom Surgery Clinic (ABN to be confirmed) is committed to protecting your personal and health information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and applicable Queensland legislation.
We collect personal and health information that is reasonably necessary to provide you with specialist oral and maxillofacial surgical care, including: name, date of birth and contact details; Medicare, DVA and health insurance details; medical and medication history; clinical notes, imaging and pathology results; referral correspondence; and billing and payment information.
Collection required by law: Certain information is collected because we are required to do so by law. For example, Medicare legislation requires us to collect your Medicare number and verify your identity for billing purposes, and AHPRA professional standards require us to maintain accurate clinical records.
What happens if you choose not to provide information: You are not obliged to provide personal information to us. However, if you choose not to provide information that is reasonably necessary for your care, we may be unable to provide treatment, process Medicare or health fund claims, or fulfil our duty-of-care obligations. We will always explain why specific information is being requested.
Your information is used to provide medical and surgical services; process billing (Medicare, DVA, private health funds); send appointment reminders via SMS, email or post; liaise with your referring practitioners and allied health providers; and, on occasion, use anonymised information for clinical research and education.
We may share your information (only where necessary) with:
Disclosure to overseas recipients: Wisdom Surgery Clinic does not store or process your personal or health information on servers located outside Australia. All software, cloud-based systems, and service providers used by the practice — including our practice management system, email, and AI scribe — store data on Australian-based servers. If this position changes in the future, we will update this policy and notify affected patients accordingly.
Where telehealth consultations are offered, we use platforms that comply with Australian privacy and security standards. Information shared during a telehealth session is treated with the same confidentiality as an in-person consultation and documented in your clinical record.
Before your first telehealth consultation, you will be asked to acknowledge: your consent to the telehealth modality; that you are in a private environment; that telehealth has inherent limitations and a face-to-face consultation may be required; and identity verification requirements.
If an AI scribe, transcription tool, or recording is used during a telehealth session, we will inform you at the start of the consultation and obtain your explicit consent before proceeding.
Where you participate in the My Health Record system and have not restricted access, Wisdom Surgery Clinic may upload relevant health information including specialist letters, referral responses, event summaries, and discharge summaries.
Queensland-specific restriction: Under the My Health Records Regulation 2012, certain categories of sensitive health information — including HIV/AIDS status and notifiable conditions under the Public Health Act 2005 (Qld) — cannot be uploaded without your explicit, informed consent.
Medicare data: Medicare encounter data will appear in your My Health Record regardless of your consent preferences with our practice. This is managed by Services Australia.
You have the right to restrict or opt out of My Health Record entirely at myhealthrecord.gov.au or by calling 1800 723 471.
Under the Privacy and Other Legislation Amendment Act 2024 (POLA), from December 2026 we are required to disclose AI or automated decision-making tools used in clinical or administrative processes.
AI Clinical Scribe — Lyrebird Health. Wisdom Surgery Clinic uses Lyrebird Health, an Australian AI medical scribe, to assist with clinical documentation. With your consent, Lyrebird may be used during consultations to transcribe the conversation and generate clinical notes for Dr Raymond's review.
Your consent is required before Lyrebird is activated. You have the right to decline at any time without affecting your care. Lyrebird is a documentation tool only — it does not make clinical decisions, diagnoses, or treatment recommendations. All notes are reviewed and approved by Dr Raymond.

In the event of a data breach likely to result in serious harm, we will notify affected individuals and the Office of the Australian Information Commissioner (OAIC) as required under the Privacy Act 1988.
You have the right to access your personal and health information held by this practice, and to request corrections. To make a request, contact us in writing. We will respond within 30 days. In some circumstances, access may be declined — we will provide reasons if this occurs.
If you believe your privacy has not been appropriately protected, please contact us in the first instance. If you are not satisfied with our response, you may contact:
Wisdom Surgery Clinic is committed to transparency in financial matters. We believe that informed financial consent is an essential part of informed medical consent.
Before any elective procedure is booked, you will receive a written fee estimate containing the following information:
| Item | Details |
|---|---|
| Surgeon's fee | Dr Raymond's surgical and consultation fees, by MBS item number |
| Hospital fee | Estimated hospital charges (subject to your health fund cover) |
| Anaesthetic fee | Estimated anaesthetic fees (charged separately by your anaesthetist) |
| Estimated Medicare rebate | Where applicable |
| Estimated health fund benefit | Where applicable — subject to your policy and waiting periods |
| Estimated out-of-pocket cost | Your likely gap, based on information available at the time |
| Prosthesis / implant costs New | Where applicable (e.g. dental implants, fixation plates) |
No-gap and known-gap schemes: New Wisdom Surgery Clinic does not currently participate in any no-gap or known-gap arrangements with private health insurers. This means there will typically be an out-of-pocket cost for surgical procedures. We will always make this clear in your written quote so there are no surprises.
Consultation fees are payable at the time of the appointment. Our team will advise you of the applicable fee before your consultation. A Medicare rebate may apply if you hold a valid Medicare card and a current referral from your GP, dentist or specialist.
Surgical fees are to be paid in full prior to the procedure date, unless otherwise agreed in writing. Receipts and tax invoices will be issued after services are rendered and sent to the contact details provided.
Eligible DVA patients are encouraged to confirm their cover with DVA before their appointment. We will liaise with DVA on your behalf where required. Appointments cannot be confirmed until written authorisation is received from DVA.
Public System Alternative: New For some procedures, treatment may also be available through the public hospital system at no out-of-pocket cost, although waiting times may apply. If you would like information about public system options for your condition, Dr Raymond or our team can discuss this with you during your consultation.
Occasionally, the final cost of your treatment may differ from the original estimate — for example, if additional procedures are required during surgery due to unexpected clinical findings. If this occurs, we will explain the change and provide an updated cost breakdown as soon as practicable. Where possible, any variation will be discussed with you before additional work is carried out.
Accounts unpaid after the due date may incur late payment charges. Outstanding accounts referred to a debt collection agency may attract additional recovery costs, including debt collection and legal fees, which will be added to the outstanding balance. We encourage you to contact us promptly if you have any difficulty meeting payment obligations.
We request a minimum of 48 hours notice for cancellations or rescheduling. Late cancellations and no-shows may incur an administrative fee, which will be communicated to you at the time of booking.
By providing your contact details, you consent to receiving the following communications from Wisdom Surgery Clinic via email, SMS, and/or post:
Documents sent to the contact details you provide will be deemed received, unless Wisdom Surgery Clinic is notified of an error in those details. It is your responsibility to keep your contact details current.
You may opt out of non-essential communications at any time by notifying our reception team. Opting out will not affect your ability to receive communications essential to your clinical care.
We do not share your contact details with third parties for marketing purposes.
As part of your care at Wisdom Surgery Clinic, clinical photographs and imaging may be taken before, during, and after your treatment. This is standard practice in oral and maxillofacial surgery and forms an important part of your clinical record.
Clinical photographs taken for the purposes of your treatment and records are collected as part of your care and are covered by your general consent to treatment. You will be asked to provide separate, specific written consent before any images are used for education, research, or marketing purposes.
You have the right to decline photography for any purpose beyond your clinical record. Declining will not affect the quality of care you receive, though in some cases it may limit certain treatment planning options — Dr Raymond will discuss this with you if relevant.
Clinical images are stored securely within your patient record and handled in accordance with our Privacy Policy and the Australian Privacy Principles. Images used for education or research are de-identified before use. Marketing images are only used with your signed written authorisation and will not be published or shared beyond the agreed scope.
Wisdom Surgery Clinic is committed to supporting you through your recovery. We provide clear after-hours guidance for all surgical patients at the time of your procedure.
Written post-operative care instructions will be provided to you before you leave following any surgical procedure. Please read these carefully — they include information about pain management, wound care, diet, activity restrictions, and what to expect during your recovery.
Please go directly to your nearest hospital emergency department or call 000 if you experience any of the following after a procedure:
For non-urgent concerns that can wait until the next business day, please leave a message with our reception team and we will respond promptly. For urgent but non-life-threatening concerns outside of business hours, our after-hours contact details will be provided to you at the time of your procedure.
Please do not use email for urgent or time-sensitive clinical matters.
